1) Download the file
2) Once opened,
fill out the "grayed" areas of the form, Continue through the form by
hitting your "Tab" key
3) Print out the "Account Deposit Card"
4) After printing out the form, use an ink pen and put your initials next
to the type of "Ownership Account" that you wish to maintain
5)
Sign bottom of page 2 in ink
6) Fold "Account Card" place in stamped
envelop and mail to address at the top of the "Account Card"

service to follow you account and track your investments will be developing each
